How to Refer a Patient
- Choose the office your patient will visit: Placentia or Huntington Beach.
- Fill in the patient’s details, the teeth and the treatment you would like us to evaluate.
- Attach radiographs with the paper clip at the bottom of the form, or send them with your patient.
- Sign and submit. The form goes securely to the office you chose.
